    If your kid (elementary and younger) has not renewed their medical form for this year, please do so by clicking the button below. This enables us to have their updated information ready should it be necessary.

  • RISING Kids Pre-Check Form

    Speed through check-in at any RISING Kids event by filling out this medical and media release form ahead of time. You don’t even need to remove your shoes, laptops, liquids or jackets! It will give us all the information we need to ensure that your child is well cared for. We update our RK Pre-Check forms once a year with all of our families at RISING. This form is good for this school year.
